Asset Tracking Updates: CA-2 and CA-3 Forms

Each department within NC State University is responsible for maintaining and safeguarding all assets. The Asset Management team has transitioned both the CA-2 and CA-3 tracking forms from static PDFs into dynamic Google Docs. This upgrade streamlines university property tracking, making NC State compliance cloud-based and collaborative. Governed by university policy REG 07.30.14 – Capital Assets – Home Use of Equipment and REG 07.30.12 – Capital Assets – Annual Equipment Inventory, these forms are the official mechanisms used to authorize university property taken home by faculty or staff for a period greater than 30 days or stored off-campus at a remote, private or leased location rather than an official university facility.

Form CA-2: Equipment Tracking Home Use Authorization

  • The purpose: CA-2 serves to document authorized home use. This requirement applies to all home equipment setups, regardless of the item’s total cost or whether it has an official asset tag.
  • The new version: The updated Google Doc format acts as a living, digital record. It tracks the asset’s complete remote lifecycle—capturing department OUC codes, securing OIT-compliant electronic approvals from department heads and logging the final verification when the equipment is returned.

Form CA-3: Storage of Assets on Privately Owned/Leased Premises

  • The purpose: CA-3 formally notifies the Asset Management team of any property residing at an off-campus facility. This record ensures that off-site university property remains accounted for during mandatory annual physical inventories.
  • The new version: The new cloud-based CA-3 template replaces the manual print-and-scan workflow. It allows departments to digitally document facility addresses, move-out timelines, expected return dates and asset tag numbers, automatically routing the data to update the university’s master equipment listings.

Please discontinue using previous versions of forms CA-2 and CA-3 for new assignments. However, existing forms remain valid for equipment that has already been assigned but not yet returned. The new forms can still be accessed through the University Controller’s Office website > Forms page.
